have sent in your RSVP please be sure to be here by 6:00 p.m.
A Note From The Nurse
We are offering a free Measles Clinic at Arbor Heights on March 12, 1998 at
10:30 a.m. Fifth grade students are required to have a 2nd Measles vaccine
before entering sixth grade. The required immunization can be administered
by your physician or community clinic.
The 5th grade students who do not show documentation for the 2nd MMR shot
have received forms to give to their parents. Please complete the forms if
your child has received the second shot with the date and signature.
If you would like your child to receive the free immunization at Arbor
Heights, please complete the registration and consent form with your child's
name and your signature. Please call me if you have any questions.
